Once ingested, the eggs enter the gastrointestinal tract where, under the influence of digestive fluids, the larvae hatch, penetrate the intestinal mucosa, and enter the portal vein system. They travel through the portal vein into the liver, where most of the larvae remain.
Within three weeks inside the body, the larvae develop into hydatid cysts. These growing cysts in the liver gradually enlarge, causing compression symptoms in neighboring organs, which can lead to obstruction, infection, and even rupture and dissemination of the cysts.
From her medical history, it appears Jiang Li had traveled to the pastoral region of Naqu, Tibet, where she may have contracted liver hydatid disease. However, as her visit was a year ago, and assuming this timeline, the disease has developed in only one year. Yet it has already eroded most of her liver and invaded the abdominal and thoracic cavities, suggesting an unusually rapid progression of the disease.

Cyst rupture into the abdominal cavity is a rare complication of liver hydatid disease. Once the cyst bursts into the cavity, the cystic fluid, which contains a large amount of foreign protein, can cause a severe allergic reaction, leading to peritonitis and anaphylactic shock.
CT imaging is the common method for diagnosing this condition, and the CT signs indicative of an acute abdominal emergency due to a ruptured liver cyst are the "serpent sign" and "water lily sign." These characteristic imaging descriptions were first detailed by European doctors in 1990 and 1991.
Jiang Li is currently in shock, and there are two potential causes: infectious shock and anaphylactic shock, which are intertwined, making treatment very difficult. Relying solely on medication is almost impossible; emergency surgery is imperative to remove the liver hydatids, the sections of liver tissue eroded by the parasite, the ruptured cysts, and the disseminated cystic fluid. Only by thoroughly eliminating the pathogen, and with adjunctive anti-shock therapy during and after the surgery, is there hope to save Jiang Li’s life.
